Use this when you had an injury that was resolved on the jobsite so you could go back to work — so the office has a correct record if it complicates later.
3 Incident / close-call (if any)
Use this if you had a close-call or kept an incident from happening. The Safety Team uses these to prevent jobsite accidents. (A report that leads to a change can earn Safety Incentive Rewards.)
